  • Goshen Medical Consent and Release Forms

    Attached are the consent and release forms (3 of them) that MUST be completed & signed by parent (all 3 forms) for any student prior to Goshen Medical staff seeing that student.   Also attached (and can be downloaded as well from this LINK) is the actual physical evaluation form must be used for sports physicals – the same form as those the Print Shop should have sent to your school back in June.  The student and parent needs to have already completed and signed the Medical History section of the Physical Evaluation form prior to being seen by the Goshen Medical staff.

    Please make sure your students and their parents are aware that the Goshen Medical staff CANNOT provide physical exams to students if they do not have all 3 of the consent and release forms signed by the parent.

     

    Your students can go to any of these locations, but they MUST have all necessary forms & paperwork completed and signed when they arrive at any of the venues in order to get the free physical exam.

     

    Please get this information out to your student-athletes.  We certainly want Goshen Medical to feel like it was worth their time and effort to bring their mobile unit out to each of our school sites.
